For twenty years I officiated high school, AAU and park district basketball games, retiring recently. For a few officiating is the focus of their occupation, while for most working as an umpire or basketball referee is an avocation. I started ref'ing to earn beer money during college, but it became a great way to stay connected to the best sports game in the universe. You need to be precise with your question. The rulebook does not define stepping through. It lays out the rules based on what can be done after you establish your pivot foot. So again, if your left foot has been established as your pivot, then you can step on your right foot and lift your left. You cannot then put down your left or drag your right.
In NFHS rules a referee should stop the game immediately for an injured player in imminent danger or the loss of contacts or glasses. Otherwise, the offense is allowed to complete their play, and then the ref can stop the game.
Administer the jump ball again. If it occurs again, issue a warning to each player, and if it happens again issue a double technical. I have never seen that.
You have the right to your vertical space. If the defender steps in your space AND makes contact, it is a foul. Generally no contact no foul.

I only had to do a couple write ups and I don't have them anymore. However, here is my advice.1) stick to facts not impressions. For example don't say I felt unsafe, instead write he threatened me if it was true. 2) include prior events that led to the final result, 3) remember that the reader was not there and will evaluate your write up with an independent, unbiased point of view.
The defender has to achieve the space before the offensive player alights or begins to step into that space. In other words the defender has to get there first, before the dribbler leaves his feet or steps towards that space.
Yes as long as it is not deemed unsportsmanlike by judgement of the referees.
